Support and Education
With Life Tech you can feel comfortable the patient, family, and caregivers are educated properly on use and care of equipment they receive for the home setting.
When dealing with medically fragile patients, proper training and education is vital for a successful hospital discharge, safe homecare environment, and to avoid unnecessary hospital readmissions.
Life Tech offers the following services for our patients and referrals to help with the transition from hospital to home:
24 hour “on call” team of 4 (RT, Technician, RN, Manager)
Early involvement thorough participation on hospital’s discharge planning team
Thorough patient, family, and caregiver training (verbal/written) both in hospital and at home
Home Environment Evaluation to assure specialized needs for life support equipment and safety are met
Coordination of community support services (electrical, fire, EMT if needed)
Immediate post discharge reassurance and support to homecare staff and family
Ongoing follow-up visits by a Respiratory Therapist to assess home care equipment
